SESSION OVERVIEW

Recent Home Health reforms have rewired our model in Value terms. PDGM, VBP, and future changes will continue to challenge Providers to remain successful, as the reforms relate directly to financial outcomes. Most agencies struggle to find PPS-level success in the Value era because the changes require ongoing modification to long-standing HH operations. This progressive presentation breaks down the reforms and outlines changes necessary, how to install and maintain them, and how to lead a successful culture change in your agency. Arnie Cisneros is the President of Home Health Strategic Management; he has 30+ years of experience as a Physical Therapist across the care continuum, and he serves as a Post-Acute Consultant for multiple Health Systems and HH Providers.  He is renowned for his adaptation of traditional Healthcare operations to address ongoing CMS reforms.

Kimberly McCormick

Executive Clinical Director

Home Health Strategic Management

Kimberly McCormick, RN/BSN, a highly accomplished clinical consultant in the Home Health industry, is Executive Clinical Director for Home Health Strategic Management. Her experience in Home Health, 8 spent as Administrator of a Home Health Agency, has given her the experience and the knowledge to provide unrivaled insight into the home care arena. Kimberly has established herself as a national expert in the area of Utilization Management and Operation Excellence in Home Health services.

              


CANCELLATIONS

Cancellations are subject to 20% handling fee. There will be NO REFUNDS issued for cancellations received within 10 business days prior to program. Cancellations must be in writing. Refunds will not be made for no-shows. However, substitutions will be allowed. Should CAHSAH® cancel the program, a full registration fee refund will be issued.


CONTINUING EDUCATION UNITS (CEUs)

Provider approved by the California Board of Registered Nursing (Provider #CEP17394) for five and one half (5.5) contact hours of continuing education. Continuing Education report will be issued to attendees once attendance has been confirmed.
